National Transitions to Adulthood - strategy for young disabled people: supplementary report - easy read
Easy read version of the supplementary report to the Transitions Strategy. It sets out actions and commitments across key policy areas, showing how work to improve transitions for young disabled people and the Transitions Strategy are delivered.

We want to:
- work with others to remove things that make transitions difficult
- find solutions that work well
- share good ideas to make policies work together
- make policies easier to understand
- make sure that everyone involved feels more confident and supported
We want a Scotland where young disabled people have control of their future.
We want to make changes so that things are better for young people.
The changes must be what young people want and need.
We will work on the actions in the Transitions Plan and the first progress update will be at the end of 2026.
