National Transitions to Adulthood - strategy for young disabled people: supplementary report - easy read
Easy read version of the supplementary report to the Transitions Strategy. It sets out actions and commitments across key policy areas, showing how work to improve transitions for young disabled people and the Transitions Strategy are delivered.
Health and Mental Health
We want a system that supports health and mental health wellbeing through the transition to adulthood:
- using transition nurses
- using health passports – these are documents that help disabled people to explain their needs and likes
- make sure that different services work together on planning
What we will do next
- work with PAMIS (Promoting A More Inclusive Society) to support more young people with complex needs in transitions by using their Digital Passport
- keep funding Spina Bifida Hydrocephalus Scotland services for young people aged 12 to 24 including their Young People’s Transitions Worker
- start a group of health organisations and Scottish Government staff to learn from the work of the clinical network for children with exceptional healthcare needs (CEN)
- work with Health Improvement Scotland to:
- decide if it is a good idea to develop transitions guidelines for health services in Scotland
- find ways for services to be made better
look at the responses to the Long-Term Conditions Framework consultation and decide:
- how the framework should change
- how the work to change things should be done
- how decisions will be made and what work should be done first
- help staff understand how to apply the GIRFE approach and tools
The GIRFE approach was developed to improve how health and social care support is delivered for people in Scotland.
