Easy read version of the supplementary report to the Transitions Strategy. It sets out actions and commitments across key policy areas, showing how work to improve transitions for young disabled people and the Transitions Strategy are delivered.
Social work and social care
Young disabled people told us that the move from children’s services to adult services is very difficult for them.
Social work and social care services include:
- assessment – ways to check what services and support people need
- care planning
- ways to make sure support is right for each person
These services must give people choice, dignity and stay the same over time.
What we will do next
work with Health and Social Care Partnerships and adult social work teams to make sure organisations work together on:
- doing transitions planning earlier
- how people connect with healthcare services and get the care and support that meets their health needs
- reviews – making sure work is going well
- make sure the way that transitions are dealt with links with:
- The Promise – a plan to make Scottish children’s care services better
- the ways that national care services are changing
- Getting It Right For Every Child (GIRFEC) – Scotland’s national plan to help children and young people get the right support from the right people at the right time
and make sure transitions are everyone’s responsibility.
- the National Social Work Agency will start by spring 2026 to support social workers and give national leadership
- support the checking and changing of the Standards in Social Work Education
- make better ways of working using information from a report called the SLIF co-design report
It will improve adult social care support, social work, and community health.
- use what we learned from the Scottish Government’s ‘Moving On’ consultation about young people leaving care to:
- support new guidance and policies
- think about how laws should change
