National Transitions to Adulthood - strategy for young disabled people: supplementary report - easy read

Easy read version of the supplementary report to the Transitions Strategy. It sets out actions and commitments across key policy areas, showing how work to improve transitions for young disabled people and the Transitions Strategy are delivered.


Introduction

A transition to adulthood is when a child grows to a young adult.

This happens over many years with many different changes.

In that time a child or young person might move from one service to another, like young people leaving school and going to college.

This Easy Read report will explain what Scottish Government work is happening to make transitions better for young disabled people in Scotland.

Each section sets out:

  • what Scottish Government and local councils will do in the future
  • how Scottish Government and local councils work with other organisations to make the Transitions plan happen

Transitions work well if people:

  • use clear and accessible information
  • connect policies between different areas

Policies are documents that explain how we want to make things happen.
