Appendix B: Example of Maternity Theory of Change Model

A table setting out an example of a Maternity Theory of Change model. It shows eight broad areas of activity with specific activities under each one collectively leading to (1) one to two year outcomes, (2) three to five year outcomes and (3) a five year plus vision. The eight activities are (1) leadership, (2) workforce knowledge and skills, (3) workforce wellbeing, (4) collaboration/power sharing, (5) organisational support, (6) screening, assessment, care and treatment, (7) physical environment and (8) research and evaluation. The diagram is a useful tool to help teams develop their change theory and articulate what parts of the system should change and in what way to achieve long term transformational change.
