Appendix E: Screening, Documentation and Care plan Implementation Driver Diagram

This diagram sets out how the ways in which person-centred, collaborative screening, documentation and care planning (where the power is shared with those with lived experience and relationship-based) can improve service users’ recovery from trauma and reduce the risk of re-traumatisation. The diagram sets out the steps that need to be taken (including primary and secondary and a number of key change ideas) to achieve this overarching aim. It highlights a range of proactive measures such as carrying out a review of the current process on screening and documentation of previous or recurring trauma.
