Ministerial Accountability Board Final Report Easy Read version
This report outlines how the Ministerial Accountability Board oversaw progress on 43 recommendations and actions arising from three Fatal Accident Inquiries of deaths in prison custody.
Commitments by the Cabinet Secretary
The Fatal Accident Inquiries made 36 recommendations. The Government also made 7 commitments. These include:
- free legal aid for families. This means free help from a lawyer
- better services for families
- changes to hold prisons responsible when someone dies
- a new look at the Fatal Accident Inquiry system. This checks if the system for looking into deaths is working properly
- making a national system for looking into deaths that happen in custody – this helps to learn from deaths that happen in prisons
