Ministerial Accountability Board Final Report Easy Read version
This report outlines how the Ministerial Accountability Board oversaw progress on 43 recommendations and actions arising from three Fatal Accident Inquiries of deaths in prison custody.
Introduction
This report talks about death in prison. This may be upsetting.
Two young people were in prison. Their names were Katie Allan and William Lindsay. William was also sometimes known as William Brown.
Katie and William died while in prison. This should not have happened. Their families have spoken about problems in prisons such as:
- Poor suicideprevention . Suicide means taking your own life. Prevention means stopping something from happening.
- Delays in Fatal Accident Inquiries (FAIs). A Fatal Accident Inquiry looks into how someone died.
- Missed chances to make things better.
A report said that changes are needed. This was agreed by the Scottish Ministers.
A board called the Ministerial Accountability Board (MAB) was set up to help with this work. The Board looked at information.
