My co-ordinator
The coordinator is a key member of My Team who will support the individual and help everyone work together to understand the person’s needs and what is important to them.
When to appoint a coordinator
A coordinator should be appointed when a person:
- needs ongoing care and support
- asks for help organising care
- has complex needs
- gets support from many different professionals, organisations and agencies
The coordinator should have a copy of the My Plan document. They:
- organise appointments and meetings
- find out more information when needed
- provide options and update My Plan if things change
- make sure everyone involved knows of changes
The coordinator will share important information about My Plan with the team to ensure everyone knows what they need to do and of any changes that happen.
Who is appointed as a coordinator?
The coordinator will be a member of the support team who has a trusted relationship with the person. They can have any role but are likely to be someone the person talks to the most often.
The coordinator must understand the individual’s needs and what is important to them around their care. If the needs change, another member of My Team can be appointed as coordinator. The individual should be told of any changes as soon as possible.
Contact
The individual receiving support should be told who their coordinator is and be provided with contact details. Alternative contact details should also be provided to enable the person to get help when the coordinator is not available.
