We are consulting on this draft updated polypharmacy prescribing guidance. 'Appropriate Prescribing - Making medicines safe, effective and sustainable 2025-2028' aims to further improve the care of individuals taking multiple medicines through the use of 7-Steps medicine reviews and promotes a holistic approach to person-centred care.

3. Acknowledgements
It has been a pleasure to chair the development of the fourth edition of Polypharmacy Guidance: Appropriate Prescribing - Making medicines safe, effective, and sustainable and to work with a team who are committed to improving outcomes for individuals. This edition has been produced by the collaborative efforts of a multidisciplinary team of healthcare professionals, academics and policy makers from across Scotland, who are already delivering polypharmacy reviews to improve the levels of appropriate prescribing and patient safety. The input of patient groups and individuals with lived experience has been invaluable.
The updating of case finding and clinical outcome indicators was undertaken by the Data and Indicators subgroup. Data relating to numbers needed to treat and adverse drug reactions have been updated. The ManageMeds polypharmacy app has been updated with colleagues from the Right Decision Service to make it more user friendly.
