Value based health and care action plan: measurement framework

This report is the first iteration of the Value Based Health and Care measurement framework. It presents updates and monitoring metrics which aim to provide an assessment of the progress with and impact of work aligned to each of the 13 actions from the value based health and care action plan.


Action 8

The Scottish Government will support people and communities to access preventative and proactive approaches to improve healthy life expectancy and reduce health inequalities.

Progress to date and future priorities

GIRFE (Getting It Right For Everyone) is Scotland’s approach to designing and delivering public services that focus on people’s needs. It was developed to improve how health and social care support is delivered for people in Scotland. Significant progress has also been made in embedding GIRFE across Scotland.

The GIRFE Team Around the Person Toolkit has been made available to all NHS Boards and Health and Social Care Partnerships to support planning and implementation. GIRFE implementation is now a formal requirement within NHS Health Board Annual Delivery Plans. It is also embedded within the NHS Scotland’s Service Renewal Framework, including through the cardiovascular disease (CVD) Risk Factors Programme. The programme has a number of active workstreams and working groups – all focused on key elements of the programme mission of reducing CVD in Scotland; with a stretch aim to reduce avoidable CVD deaths by 20% in 20 years.

As part of the Scottish Government’s work to monitor progress on Health and Social Care Reform, plans are in place to report on Health and Social Care Reform portfolio delivery including progress in portfolio level outcomes and indicators to demonstrate delivery towards the Health and Social Care vision. This will capture progress of Scotland’s Health and Social Care Service Renewal Framework and Population Health Framework, including monitoring progress against their respective priorities and actions. Aligned to the aims of this Action, the Population Health Framework aims to improve Scottish life expectancy whilst reducing the life expectancy gap between the most deprived 20% of local areas and the national average by 2035. As these plans develop, the Scottish Government will consider how measurement of Action 8 from the VBH&C action plan can draw on Health and Social Care Reform monitoring and reporting to ensure coherence and alignment.

Monitoring metrics

The monitoring metrics for Action 8 draw on survey data from health and care professionals, survey responses from the Fourteenth Citizens’ Panel and national demographic statistics. Together, these data sources provide insight into how organisations are perceived to promote a culture of prevention and address racialised inequalities, public perceptions of preventive discussions in recent healthcare interactions and offer wider context on population health.

Metric 8.1: 51% of health and care professionals surveyed agreed that their organisation promotes a culture of prevention in health and care.

Metric 8.2: 43% of health and care professionals surveyed agreed that their organisation actively works to address racialised inequalities in health and care.

Metric 8.3: When asked to think about the last healthcare professional they saw within

NHS Scotland, 63% of Citizen’s Panel respondents agreed that they were involved in discussing what they can do to promote good health and prevent ill health in the future.

Metric 8.4: Life expectancy at birth in Scotland was 81 years for females and 77 years for males in 2022-2024.
