SMR04 - mental health (psychiatric) hospital inpatients and day cases

SMR04 is an episode-based patient record relating to all inpatients and day cases admitted to and discharged from Mental Health specialties. SMR04 consists of two parts: 

  • Admission information including status on admission and diagnosis information. 
  • Discharge information (which includes a copy of the admission details) including diagnosis information. 

About 32,000 admission, and 32,000 discharge records are generated each year. Analyses can be carried out on admissions, discharges and residents from the available data.

General Acute Inpatient and Day Case - Scottish Morbidity Record (SMR01) 

In addition to the SMR04 recording scheme for inpatients and day cases cared for in mental health specialties, activity for patients with mental and behavioural disorders cared for in acute (general hospital) specialties is recorded on the SMR01 scheme.

The Public Health Scotland report; Mental Health Inpatient Activity, presents information on patients with mental health problems or learning disability who have been cared for as in both psychiatric and general acute specialties in Scottish hospitals.

SMR00 and ISD(S)1 – outpatient attendances 

Psychiatric outpatient attendances are recorded on the SMR00 (patient level information on outpatient appointment/attendance record) and ISD(S)1 (aggregated summary statistics on activity in hospitals and other health care settings in Scotland) schemes. Diagnostic information is not available from ISD(S)1 or from SMR00.

Mental Health Inpatient Activity in Scotland 

Mental Health Inpatient Activity in Scotland presents information on patients with mental health conditions treated as inpatients in Scottish NHS inpatient facilities. Data are included for patients receiving inpatient care in psychiatric facilities (Scottish Morbidity Record 04, SMR04) and patients receiving care as inpatients and day cases in non-psychiatric (acute) hospitals where the patient was recorded as having a mental health condition (Scottish Morbidity Record 01, SMR01)

The Scottish Suicide Information Database (ScotSID) 

The Scottish Suicide Information Database (ScotSID) was established in 2008. It provides a central repository for information on all probable suicide deaths in Scotland. The database covers demographic information, contact with specified health services, and related health data. ScotSID is formed by the linkage of death records with several other datasets (e.g. SMR01, SMR04, and part of SMR00).

The Scottish Public Health Observatory  

The Scottish Public Health Observatory (ScotPHO) is a collaboration led by Public Health Scotland, and includes the Glasgow Centre for Population Health, the National Records of Scotland and the Scottish Learning Disabilities Observatory. 

The ScotPHO website includes a mental health topic with extensive information on the mental health policy landscape in Scotland and a range of data sources and external sources of quality-assured evidence touching on different aspects of mental health conditions and specific populations. Scottish Government Mental Health Resources  

The Scottish Health Survey 

The Scottish Health Survey (SHeS) is an annual survey that provides a detailed picture of the health of the Scottish population in private households and is designed to make a major contribution to the monitoring of health in Scotland. SHeS collects a range of data points relating to mental health and wellbeing, covering: 

  • mental wellbeing (Warwick-Edinburgh Mental Wellbeing Scale-WEMWBS) for adults 
  • risk of psychiatric disorder (General Health Questionnaire 12-GHQ-12) for adults 
  • stress at work for adults 
  • loneliness for adults 
  • child strengths and difficulties questionnaire (SDQ)

Health and Care Experience Survey 

The Scottish Government’s Health and Care Experience (HACE) survey asks about people’s experiences of, accessing and using their general practice and out of hours services, aspects of care and support provided by local authorities and other organisations, caring responsibilities and related support. Inpatient Experience Survey 

The Scottish Government’s Inpatient Experience Survey aims to provide local and national information on the quality of health and care services from the perspective of those using them. Full details can be found here: Inpatient Experience Survey - gov.scot 

Learning Disability Statistics Scotland 

The Scottish Commission for People with Learning Disabilities (SCLD) previously published statistics about adults with learning disabilities who are known to local authorities in Scotland. All local authorities in Scotland collect information on the numbers of people with learning disabilities and/or autism in their area. A range of data was collected on housing, care, employment, and education. Due to issues with returns, data protection and prioritisation during the pandemic, this data has not been published since 2019.  Scottish Care Home Census 

 The Scottish Care Home Census (SCHC) has been running in its current form since March 2003. It provides the Scottish Government, Local Authorities, the private and voluntary sector with information on all registered care homes in Scotland, allowing them to monitor and plan services for the future. The results are published annually by Public Health Scotland.  Mental Welfare Commission for Scotland 

The Mental Welfare Commission for Scotland aims to ensure that care, treatment and support are lawful and respect the rights and promote the welfare of individuals with mental illness, learning disability and related conditions. The Commission produces annual statistical monitoring reports based on an independent overview of the operation of the Mental Health (Care & Treatment) (Scotland) Act 2003 and the Adults with Incapacity (Scotland) Act 2000. This is a legal requirement and is embedded in legislation.

The Commission receives notifications of most interventions under the Mental Health (Care & Treatment) (Scotland) Act 2003. It uses these to report on how the Act is used and to monitor trends over time and geographical variations. A range of data is held, including emergency detentions and short-term detentions under the Act, for which certificates are issued. These detentions include cases of formal admission to hospital, about which each NHS Board notifies the Commission.

Where to find data on mental health and mental health services in the UK 

Because health is a devolved policy area, data is collected and published separately for each UK nation.
