Long-term conditions

Long term conditions are any physical illness or mental illness expected to last a year or more.

A framework is a work plan.

Scottish Government policy must support the needs of all people living with long term conditions.

But we must decide how best to spend the money we have.

We know that many people living with long term conditions will need the same types of care and support.

These might be things like physiotherapy or checking in on their condition.

We want to make sure everyone who needs those services can get them.

We know we must also have some services that are specific to each condition when needed.

We must look at ways to make sure different services work well together.

We want to make sure we get better outcomes for patients.
