Interim National Care Service Advisory Board: Advice to Scottish Ministers and Council Leaders - Coming Home

The Interim National Care Service Advisory Board identified Coming Home as a priority theme for their consideration. The advice and recommendations have been prepared for Scottish Ministers and Council Leaders to help drive improvement and ensure consistency across Scotland.


Advice to Health Boards

The interim Board notes that there has been some positive movement recently in relation to reductions in the number of people delayed in hospital and in inappropriate out of area settings. The Dynamic Support Register also identifies people at risk of community placement breakdown, which is a key reason for admission to hospital or other inappropriate settings.

In line with the Population Health Framework, the interim Board recommends that health boards work with local authorities and health and social care partnerships to prevent placement breakdown including:

  • using the Dynamic Support Register to target preventative action for those who are at risk of placement breakdown or where risk is increasing, making effective use of review meetings.
  • making full use of tools to support prevention and early intervention, such as Learning Disability Annual Health Checks, which help to inform effective healthcare and support in the community.
