Reform case studies

The following three examples of case studies show how current health and social care reform initiatives are helping to improve outcomes and service efficiency, and relieve pressure on health and social care services. These case studies are good examples of where we are looking to take a community first and preventative approach, in line with our vision. Health and social care case study 1: Community Glaucoma Service

  • demonstrates how the flagship Community Glaucoma Service helps to improve outcomes and relieve pressure on waiting lists
  • notes the benefits of shifting the balance of care to the community sector and relieving pressure on out of hours and urgent care services, leading to improved cost effectiveness

Health and social care case study 2: Hospital at Home, older people and acute adults

  • provides a practical example of how we are providing care for people at or as close to home as possible to best support their recovery and longer-term outcomes
  • demonstrates that continued investment emphasises our commitment to care closer to home and has enabled more areas than ever to develop Hospital at Home services, including some of the most remote and rural areas of the country

Health and social care case study 3: GIRFE: Using co-design to create a holistic person centred, preventative practice model

  • Getting It Right For Everyone (GIRFE) puts the person at the centre of all decision making with a joined-up, coherent and consistent multi-agency approach regardless of the support needed at any stage of life
  • the case study focuses on Older People and Frailty, using the “Team Around the Person” toolkit to support “A”, who requested to receive rehabilitation care in her home after hospitalisation for falls which led to reduced mobility and increased frailty
