Information requested

Request for information 1:

Any form/application/documentation that must be completed by Adult Disability Payment recipients for Planned Award Reviews. Please provide a sample form/application/document that must be completed by recipients.

Request for information 2:

Any other steps or actions that must be completed by recipients of Adult Disability Payment for Planned Award Reviews. Please provide an example of the steps and actions that need to be completed by recipients including any internal guidance documents on the review process for Social Security Scotland staff.

Request for information 3:

The number of Change of Circumstance reviews which are instigated via a recipient’s notification of a change of circumstances compared to the number of Change of Circumstance reviews triggered by Social Security Scotland being independently informed of a change of circumstance (other than from the recipient of ADP). Please provide this figure for the financial year of 2024-25.

Request for information 4:

Please find attached a copy of the Review Pack which clients receive when they are due for a planned review.

Please find attached internal guidance that Social Security Scotland staff use when clients are coming up for a review.

Response to information request 3:

We have interpreted your question to relate to unscheduled reviews.

Notification of a change of circumstances will not necessarily prompt a review.

Below are the figures for the number of unscheduled reviews completed in the financial year 2024-2025:

  • Client initiated: 16,644
  • Agency initiated: 669
  • Case transfer: 11,630
