I enclose a copy of all of the information you requested.  For point 10, the table below displays the number of businesses which had the scheme breach reason listed applied to their application(s).  As more than one scheme breach can be applied to an application, the same business may appear against more than one scheme breach reason.  Equivalent data for scheme year 2018 is not yet available as the application window has just recently closed (30th November).
