Statistical and modelling methods are described throughout. Unless stated otherwise analyses were carried out using the R statistical environment ^[27] (versions 2.14.1 and 2.15.2) utilising, as required, the supplementary R packages boot (version 1.3-7) ^[28] , MASS ^[29] (version 7.3-22) and VGAM ^[30] (version 0.9-1).
