The National Transitions to Adulthood strategy for young disabled people was published in June 2025. This supplementary report sets out current actions and future commitments across key policy areas, demonstrating how work to improve transitions for young disabled people is delivered in practice.
Next Steps
We recognise the current landscape of policies and support for transitions can feel fragmented, complex, and difficult to navigate. For young disabled people and their families, this can add unnecessary stress at what should be a time full of possibility and hope. That is why we are committed to progress on Priority 6: Connecting to a Wider Policy Landscape. We are determined to work together to build greater clarity, coherence, and confidence across the system.
This report is a commitment to removing barriers, aligning policies, and ensuring that everyone involved in supporting transitions has the information, guidance, and shared understanding they need. We will work collaboratively to support, champion good practice, and co-design approaches that make sense in real life, not just on paper.
Our vision is ambitious: a Scotland where young disabled people in Scotland feel confident in their transition to adulthood and are empowered and supported to control their own path to success. We are confident in the strengths of our communities, professionals, and systems, and we are clear about the work still to be done. Through continued commitment, partnership, and accountability, we are determined to deliver real and lasting change.
To young people, families, and the professionals who support them: we hear you, we stand with you, and we are committed to creating a system that works for you — now and for the future. Throughout this document lived experience has been a pillar of how we recognise the value and need for this work to benefit the lives of young disabled people in Scotland.
Our next step is to begin implementation of the Transitions Strategy working towards our first progress report by the end of 2026.
