Coming Home Action Plan 2026 - Easy Read Summary
The action plan provides an update on progress to date, addresses the outstanding recommendations from the Coming Home Implementation Report, and sets out further actions required to achieve the Coming Home vision and mission.
What this will achieve
The main aims of this action plan are listed below:
- People should be able to live where they choose.
- Families should be included when planning housing and care for people with complex needs.
- Services should work well together.
- People should not get stuck in hospital.
- Everyone should be treated with dignity and respect.
