Coming Home Action Plan 2026 - Easy Read Summary

The action plan provides an update on progress to date, addresses the outstanding recommendations from the Coming Home Implementation Report, and sets out further actions required to achieve the Coming Home vision and mission.


Human Rights, Lived Experience, and Next Steps

Human Rights

The Coming Home action plan is based on human rights.

This means that people with learning disabilities must be involved in decisions.

The plan says that people must be treated fairly.

The plan says that people with learning disabilities should always be treated with respect.

The plan says that people with learning disabilities should have choice and control over their lives.

Lived experience

People with lived experience and their families will be asked to do the following things:

  • Be part of a new Lived Experience Hub which is a group where you can
  • Help design solutions.
  • Help check the action plan is working.
  • Help make sure the voices of people with learning disabilities are heard across the country and locally.

How the plan will be monitored

New groups will be set up to make sure the action plan is delivered.

A Coming Home Strategic Delivery Group will be set up to supervise delivery of the action plan

A Coming Home Leads Group will also be set up with senior people who work in the local authority areas on coming home

There will also be regular meetings between COSLA and the Minister for Social Care and Mental Wellbeing
