Coming Home Action Plan 2026 - Easy Read Summary

The action plan provides an update on progress to date, addresses the outstanding recommendations from the Coming Home Implementation Report, and sets out further actions required to achieve the Coming Home vision and mission.


What the Action Plan Will Do Next

The things the action plan will do next are listed below:

Improve support planning

Keep making the Dynamic Support Register better.

Make sure it is used to take action, not just gather information.

Help local areas fix problems like delays and poor communication.

Create a National Support Panel

Create a panel which will offer expert help to local areas and help solve complex cases that people and services are stuck on.

This panel will also check whether people are getting the right support.

This idea is being tested out in one local area first to see how it works.

Strengthen community services

The plan will help staff learn important skills.

These are:

  • Understanding complex needs.
  • Supporting people safely.
  • Reducing restrictive practices.
  • Working with families.

A national training guide has been made to support this.

Improve housing options

The plan will help local areas understand the housing needs of people leaving hospital.

The plan will provide guidance on different types of housing options available to people.

The plan will look at making a housing advice service for staff.

The plan also says that it will work with the Scottish Government’s national housing programme.

Commissioning (planning and buying support)

The plan will help local areas to work together and share examples of good practice.

The plan will explore different funding options.

The plan will also support services to make sure they always meet individual needs and protect human rights.
