Coming Home Action Plan 2026 - Easy Read Summary
The action plan provides an update on progress to date, addresses the outstanding recommendations from the Coming Home Implementation Report, and sets out further actions required to achieve the Coming Home vision and mission.
What Has Already Been Done
Some of the main things in the action plan that have already been done are listed below:
Dynamic Support Register
The Dynamic Support Register has been introduced.
The Dynamic Support Register is a list of people across Scotland who are in hospital or stuck waiting to go home from hospital.
It also lists people who are in an out-of-area placement.
An out-of-area placement is when a person can only get the support they need if they move a long way from home and family.
It also lists people who are at risk of going into an out-of-area placement because the support in their local area could break down.
This list helps local areas plan support and helps services make sure people are not in hospital for longer than they need to be.
Peer Support Network
The Peer Support Network is a Group of professionals who meet regularly.
They share ideas on how to make sure people with learning disabilities do not stay in hospital for too long and are able to live in their own local areas.
They talk about any problems they have had in their work supporting people with learning disabilities.
They also tell each other about the things they do that work well.
This helps make support better for people with learning disabilities and complex needs.
Short Life Working Group
The Short Life Working Group is a group of experts who held meetings to agree on the most important things to do next and worked together to write the action plan.
Redesigning inpatient (hospital) services
Work has started to understand how many learning disability beds are needed in hospitals in Scotland.
Work has also started to make sure people only stay in hospital when they need treatment and they get home from hospital quickly and safely.
People in hospital 10+ years
31 people have been in hospital for over 10 years.
Work is being done with local areas to help them move these people back into the community.
