Coming Home Action Plan 2026 - Easy Read Summary

The action plan provides an update on progress to date, addresses the outstanding recommendations from the Coming Home Implementation Report, and sets out further actions required to achieve the Coming Home vision and mission.


What Coming Home Is About

Coming Home is for people in Scotland who have a learning disability and have complex support needs.

It is about making sure people can live safely in their own local area and not in hospital or far from home.

The name Coming Home comes from a report that looked at why people were living in hospital or far from home.

Why this action plan is needed:

Some people with learning disabilities have been living in hospital for too long.

Some people with learning disabilities have been living far away from home.

Some people with learning disabilities have been living in places that do not meet their needs.

This is not okay.

People have the right to:

  • Live in their community.
  • Live close to family and friends.
  • Have choice and control in their lives.
  • Get the support they need.
