Cardiovascular disease prevention scheme - directed enhanced service – final uptake 2025-2026

Management information showing the number of patients receiving Cardiovascular Disease (CVD) risk factor checks and a clinical review appointment from their General Practitioner in Scotland, funded through the CVD prevention scheme Directed Enhanced Service.


This publication includes management information provided by Public Health Scotland (PHS) from local GP systems showing the latest uptake of the CVD DES. PHS receive data extracts at aggregated practice level which limits the data quality checks that can be carried out.

Of the 880 GP Practices in Scotland, 847 opted into the CVD DES arrangements. Of these, 836 provided data for these outputs. These practices represent 96% of the patients registered with a practice in Scotland.

Data was extracted on the basis of where a patient was registered at the time of extraction which may differ from where the DES was carried out. Patients who were no longer registered with a general practice in Scotland were not counted.

Area deprivation is defined using the Scottish Index of Multiple Deprivation (SIMD), with deprivation levels calculated based on a patient’s address details as registered with their General Practice.

Risk of cardiovascular disease is identified using the ASSIGN risk score. ASSIGN estimates an individual’s 10-year risk of developing cardiovascular disease in Scotland.
